National use of chronic disease management services

This exploratory analysis reflects the number and proportion of health check patients who received a chronic disease management service in the same year as their health check. The chronic disease management service may have occurred before or after the health check.
Latest data
Among health check patients in 2023:
- 30% (or 78,300) of health check patients had a chronic disease management service in the same year.
- That proportion was strongly related to age, with 77% of patients aged 75 or older having a chronic disease management service in the same year, compared with 5% of patients aged 0–4.
